Background to chronic disease management item numbers

A GP management plan (GPMP, item 721) is an item for patients with a chronic (one that has been, or is likely to be present for at least six months) or terminal medical condition, who will benefit from a structural approach to management of their care needs.

It is necessary to document the patient’s needs, goals, patient’s actions, treatment/services and a review date, by completing the GPMP document.

  • A team care arrangement (TCA) is an item for patients with a chronic or terminal medical condition and who require ongoing care from a multidisciplinary team of their GP and at least two other healthcare providers.
  • The patient’s agreement to share relevant information about their medical condition with the proposed providers must be obtained.
  • It is important to document the goals, the collaborating providers, the treatment/services they have agreed to provide, any actions to be taken by the patient and a review date, by completing the TCA document.
